Functional Medicine vs. Conventional Medicine: Which Is Right for You?

- Conventional medicine asks what disease you have; functional medicine asks why it is happening in you.
- Conventional care is the right and only choice for emergencies, infection, cancer, and advanced organ disease.
- Functional care adds the most for chronic multi-system symptoms with 'normal' labs, metabolic dysfunction, hormone change, and prevention.
- Most patients do best with both: conventional rigor for diagnosis and safety, functional depth for the upstream drivers.
I trained in both worlds. I am double board-certified in Family Medicine and Preventive Medicine, and I hold special certifications in Functional and Lifestyle Medicine. So when a patient asks me whether functional medicine is "better" than conventional medicine, my honest answer is: they answer different questions, and you almost certainly need both.
The woman who arrives in my telehealth practice after four years of fatigue, a normal TSH, and three ten-minute visits does not need conventional medicine to be wrong. She needs someone to spend an hour on her history, order a wider panel, and address the drivers no one has time to look at in a standard visit. The man with crushing chest pain needs an emergency department, a catheterization lab, and a cardiologist β not a nutrigenomics panel.
This guide lays out how the two models actually differ, where each one is strongest, and how to decide which door to walk through.
The Core Difference: Two Different Questions
Conventional medicine is organized around the question "What disease does this patient have, and what is the standard treatment for it?" It is diagnosis-first and protocol-driven. That structure is what makes it extraordinarily good at acute illness, infection, trauma, surgery, and advanced disease β situations where the right answer is known, standardized, and needs to be delivered fast.
Functional medicine is organized around the question "Why is this happening in this particular person, and what upstream factors can we change?" It is systems-first and pattern-driven. That structure is what makes it useful for the chronic, multi-symptom, "your labs are normal" presentations that fill primary care waiting rooms and rarely resolve in them.
Neither question is the wrong question. They are just different tools.

Where Conventional Medicine Is Clearly Superior
I want to be direct about this, because functional medicine marketing often is not.
- Emergencies. Chest pain, stroke symptoms, severe bleeding, anaphylaxis, sepsis, suicidal ideation. Call 911. There is no functional workaround.
- Acute infection. Pneumonia, pyelonephritis, cellulitis, appendicitis. Antibiotics and surgery save lives; herbs do not.
- Cancer care. Oncology is protocol-driven for a reason β the protocols come from large randomized trials. Functional medicine belongs alongside oncology as supportive care, never instead of it.
- Advanced organ disease. End-stage kidney disease, heart failure, type 1 diabetes, autoimmune disease requiring immunosuppression. These need specialists and pharmaceuticals.
- Anything that needs a hospital. Imaging, surgery, transfusion, monitored care.
If a practitioner ever tells you to stop a prescribed medication without coordinating with the prescriber, that is a red flag regardless of what letters follow their name.
Where the Functional Approach Adds the Most
The functional model earns its place with the patients conventional visits are not designed to serve: the ones with several symptoms across several systems, unremarkable labs, and no single diagnosis.
- Fatigue with "normal" labs. A TSH of 3.8 sits inside the reference range and outside the optimal range. So does a ferritin of 22, a vitamin D of 28, or a fasting insulin of 14. Screening asks whether you have disease; functional interpretation asks how far from optimal you are.
- Metabolic dysfunction before diabetes. Fasting insulin, HOMA-IR, ApoB, triglyceride-to-HDL ratio, and continuous glucose data catch insulin resistance years before an A1c crosses 6.5%.
- Perimenopause and hormone changes. Symptoms often start well before FSH or estradiol look abnormal on a single draw.
- Gut symptoms without a diagnosis. Bloating, irregularity, and food reactivity after a normal endoscopy.
- Prevention with a family history. When a parent had an MI at 55, "your cholesterol is fine" is not a plan.
How the Two Models Handle the Same Patient
Consider a 46-year-old executive with fatigue, 18 pounds of new weight, poor sleep, and low libido.
Conventional path: a 12-minute visit, a CBC, CMP, TSH, and lipid panel. All within range except borderline LDL. The plan is diet and exercise advice, possibly a statin, possibly an SSRI, and a recheck in a year.
Functional path: a 60β90 minute history covering sleep architecture, work stress, alcohol, training load, and diet timing. Testing adds fasting insulin, hs-CRP, ferritin, vitamin D, a full thyroid panel with antibodies and Free T3, morning total and free testosterone, and ApoB. The plan targets whatever the data shows β protein and resistance-training targets, sleep and circadian correction, nutrient repletion, and medication where it is genuinely indicated, with a 90-day recheck.
Same patient. Different question, different depth, different follow-up cadence.
The Honest Trade-Offs
Cost. Functional medicine is usually direct-pay, including in my practice. Longer visits and broader testing cost more up front. That is a real trade-off, not a detail to gloss over.
Evidence maturity. Individual interventions in functional medicine β omega-3s, selenium for Hashimoto's, resistance training for insulin resistance β often have solid trial data. The bundled, individualized protocol as a whole has far less. A good functional clinician tells you which parts are well supported and which are reasonable but provisional.
Practitioner variability. "Functional medicine" is not a protected term. Physicians, nurse practitioners, chiropractors, and health coaches all use it. Verify licensure and board certification before you spend anything.
Time. Root-cause work takes 3β12 months of consistent effort. If you want a prescription today, this is not the faster route.
How to Choose: A Practical Filter
Choose conventional care first when the problem is acute, severe, rapidly worsening, or clearly matches a known diagnosis with an established treatment.
Choose functional care when you have chronic symptoms that cross systems, normal labs, no clear diagnosis, and a goal of prevention or optimization rather than crisis management.
Choose both β which is what most of my patients actually need β when you have a diagnosis being managed conventionally and you also want the upstream drivers addressed so the diagnosis stops progressing.
What to Ask Before You Commit
1. What is your medical license and board certification?
2. What does the initial visit cost, and what does testing typically add?
3. Which of your recommendations are supported by randomized trials, and which are clinical judgment?
4. Will you coordinate with my primary care physician and specialists?
5. What does follow-up look like, and how will we measure whether this worked?
6. Under what circumstances would you refer me out?
A clinician who answers all six comfortably is practicing good medicine, whatever label is on the door.
The Bottom Line
Functional medicine is not an alternative to conventional medicine. It is a different lens applied to the same physiology β one built for the chronic, multi-system complaints that a 12-minute visit cannot accommodate. Conventional medicine remains the right and only answer for acute and advanced disease.
The best care I can give a patient uses both: conventional rigor for diagnosis and safety, functional depth for the "why."
