I had a patient come in last year with a complaint list that read like a medical textbook: muscle cramps, insomnia, heart palpitations, anxiety, constipation, and migraines. She'd seen four different specialists. The cardiologist cleared her heart. The neurologist prescribed a migraine medication. The gastroenterologist recommended fiber. The psychiatrist added an anti-anxiety medication to her growing list.

Nobody had checked her magnesium. Not properly, anyway. Her serum magnesium (the standard blood test) came back "normal" at 2.1 mg/dL. But when I ordered an RBC magnesium level, the intracellular measurement that actually reflects tissue stores, she was profoundly deficient. Every single symptom she was experiencing could be explained by one mineral.

This isn't a rare case. It's an epidemic hiding behind "normal" lab values.

The Scale of Magnesium Deficiency

Magnesium is required for over 600 enzymatic reactions in your body. It's involved in energy production, DNA repair, muscle and nerve function, blood sugar regulation, blood pressure control, sleep, and neurotransmitter synthesis. It is arguably the single most important mineral for human health.

And we're not getting enough. Studies estimate that 50-68% of American adults consume less than the recommended daily amount of magnesium. The reasons are structural: modern soil depletion has reduced magnesium content in food by 25-80% over the past century, water treatment removes naturally occurring magnesium, and the shift toward processed foods (which are stripped of magnesium during refining) has dramatically reduced dietary intake (1).

On top of inadequate intake, modern life actively depletes magnesium. Chronic stress causes magnesium excretion through the kidneys. Caffeine, alcohol, and refined sugar all increase magnesium loss. Common medications, including proton pump inhibitors, diuretics, and certain antibiotics, deplete magnesium stores. High-intensity exercise increases magnesium demand (2).

The result: most people are running on depleted magnesium without knowing it, and the standard blood test doesn't catch it.

⚠️ Caution

Standard serum magnesium tests are nearly useless for detecting deficiency. Only 1% of your body's magnesium is in the blood; 99% is in cells and bones. You can be severely magnesium-depleted with a "normal" serum level. Always request RBC (red blood cell) magnesium for an accurate assessment. Optimal range is 5.0-6.5 mg/dL.

What Magnesium Deficiency Does to Your Body

Because magnesium is involved in so many biochemical processes, deficiency creates a cascade of symptoms that are typically treated as separate, unrelated conditions.

Sleep and the Nervous System

Magnesium activates the parasympathetic nervous system, the "rest and digest" mode that prepares your body for sleep. It does this by binding to GABA receptors (your brain's primary calming neurotransmitter) and by regulating melatonin production. Magnesium deficiency leaves the nervous system in a state of chronic excitability: difficulty falling asleep, light and easily disrupted sleep, nighttime muscle cramps and restless legs, and an inability to "turn off" the brain at bedtime (3).

This is why magnesium supplementation is one of the first interventions I recommend for patients with insomnia, even before considering sleep hygiene changes, because without adequate magnesium, the nervous system literally cannot downshift.

Heart and Cardiovascular Function

Magnesium is essential for maintaining normal heart rhythm. It stabilizes cardiac cell membranes and regulates the flow of calcium and potassium into heart cells. Deficiency can cause palpitations, arrhythmias (including premature ventricular contractions, or PVCs), and elevated blood pressure. Meta-analyses consistently show that higher magnesium intake is associated with reduced risk of cardiovascular disease, heart failure, and stroke (4).

Many patients I see with unexplained palpitations have underlying magnesium deficiency. In several cases, supplementation alone resolved symptoms that had prompted ER visits and cardiology referrals.

Stress, Cortisol, and Anxiety

Magnesium and stress exist in a vicious cycle. Stress causes magnesium depletion (cortisol drives renal excretion), and magnesium depletion amplifies the stress response (by increasing cortisol production and HPA axis reactivity). Supplementation with magnesium has been shown to reduce subjective anxiety, lower cortisol levels, and improve HRV (heart rate variability), a marker of stress resilience (5).

Blood Sugar and Insulin Resistance

Magnesium is a cofactor for insulin signaling. Without adequate magnesium, cells become less responsive to insulin, promoting insulin resistance and eventually type 2 diabetes. Studies show that each 100mg increase in daily magnesium intake is associated with a 15% reduction in type 2 diabetes risk (6). For men over 40 with expanding waistlines or rising blood sugar, magnesium status should be one of the first things assessed.

Hormones

Magnesium influences virtually every hormonal pathway. It supports healthy testosterone production in men (deficiency is associated with lower free testosterone). It modulates estrogen metabolism in women. It's essential for thyroid hormone conversion (T4 to active T3). And it regulates the HPA axis, governing your body's entire stress hormone cascade.

Muscle Function and Pain

Magnesium controls muscle contraction and relaxation. Deficiency results in chronic muscle tension, cramps, spasms, tension headaches, and migraines. Magnesium also modulates NMDA receptors involved in pain signaling, which is why deficiency can lower pain thresholds and contribute to chronic pain conditions.

Choosing the Right Form of Magnesium

Not all magnesium supplements are created equal. The form determines both absorption and where in the body magnesium is most active.

Magnesium glycinate: Best for sleep, anxiety, and general repletion. Glycine itself has calming properties, and this form is well-absorbed with minimal GI side effects. This is my default recommendation. Typical dose: 200-400mg before bed.

Magnesium threonate (Magtein): The only form shown to cross the blood-brain barrier and increase brain magnesium levels. Best for cognitive function, brain fog, and memory. Studied at 1,500-2,000mg of magnesium threonate daily (providing about 144mg elemental magnesium).

Magnesium taurate: Preferred for cardiovascular applications, particularly arrhythmias and blood pressure. Taurine itself supports cardiac function. Typical dose: 200-400mg daily.

Magnesium citrate: Good absorption, but at higher doses has a laxative effect. Useful for patients with constipation. Not ideal as a primary repletion strategy.

Magnesium oxide: Poorly absorbed (4% bioavailability). Despite being the most common form in cheap supplements, it's essentially useless for correcting deficiency. Avoid it.

How to Optimize Your Magnesium Status

Test Properly

Request an RBC magnesium level, not serum magnesium. Optimal range is 5.0-6.5 mg/dL. Most labs report a "normal" range of 4.2-6.8, but functional optimal is the upper half of that range.

Eat Magnesium-Rich Foods

The best dietary sources include dark leafy greens (spinach, Swiss chard, kale), pumpkin seeds (the single richest food source at 150mg per ounce), dark chocolate (70%+ cacao), almonds and cashews, black beans and lentils, avocados, and wild-caught fish. Aim for at least 400-600mg daily from food, though supplementation is typically needed to reach optimal levels given modern soil depletion.

Supplement Strategically

Most adults benefit from 200-400mg of supplemental magnesium daily, in addition to dietary sources. Start at 200mg and increase gradually, since rapid increases can cause loose stools (your tolerance will build). Take magnesium with food for better absorption, and separate it from zinc, iron, and calcium supplements by at least 2 hours, as these minerals compete for absorption.
