Here's the uncomfortable truth: standard lab panels were designed to detect *disease*, not optimize *health*. The ranges labeled "normal" on your lab results represent the middle 95% of the American population — a population where 88% have at least one marker of metabolic dysfunction (1). - Ferritin at 15 ng/mL is "normal" in many labs — but many women with ferritin below 30 experience significant fatigue, hair loss, and brain fog
"Normal" lab ranges tell you where disease begins. Functional medicine optimal ranges tell you where health thrives. The difference can be years of unnecessary suffering.
What Your Annual Physical Actually Tests
A typical annual physical includes a fairly limited panel:
- Complete Blood Count (CBC): Red and white blood cell counts, hemoglobin, hematocrit
- Basic or Comprehensive Metabolic Panel (BMP/CMP): Glucose, electrolytes, kidney function, liver enzymes
- Lipid Panel: Total cholesterol, LDL, HDL, triglycerides
- TSH: A single thyroid marker
- Maybe: Hemoglobin A1c (if diabetic risk factors exist)
That's it. No inflammation markers. No insulin levels. No comprehensive thyroid assessment. No hormones. No micronutrient status. No advanced cardiovascular risk markers. It's like judging a building's structural integrity by looking at the paint.
The Comprehensive Functional Medicine Panel
Here's what I run on virtually every new patient. This isn't excessive testing — it's the minimum needed to understand what's actually happening in your body. I've organized it by system so you can see what each test reveals.
Metabolic Health & Blood Sugar
Standard panels check fasting glucose and maybe A1c. But insulin resistance — the precursor to type 2 diabetes, heart disease, PCOS, and Alzheimer's — develops years before glucose becomes abnormal. By the time your fasting glucose hits 100, insulin has been elevated for potentially a decade.
What to test:
- Fasting insulin (optimal: 2-6 μIU/mL): The single most important metabolic marker most doctors never order. Elevated insulin is the earliest detectable sign of metabolic dysfunction
- Fasting glucose (optimal: 75-85 mg/dL): Standard test, but interpreted with tighter optimal ranges
- Hemoglobin A1c (optimal: 4.8-5.2%): Reflects average blood sugar over 90 days
- HOMA-IR (optimal: below 1.0): Calculated from fasting insulin and glucose; the gold standard for assessing insulin resistance
- C-peptide: Measures actual insulin production from the pancreas; helps distinguish insulin resistance from other causes
I consider fasting insulin the single most underordered lab in medicine. A fasting insulin above 7 μIU/mL indicates early metabolic dysfunction — even when glucose and A1c remain "normal." This marker can identify problems 10-15 years before a diabetes diagnosis.
Comprehensive Thyroid Panel
Your doctor orders TSH. If it's "normal," they move on. But TSH alone is like checking the thermostat without measuring the temperature in the room.
What to test:
- TSH (optimal: 0.5-2.0 mIU/L): Note the tighter optimal range vs. the standard 0.4-4.5
- Free T4 (optimal: 1.0-1.5 ng/dL): The inactive thyroid hormone — what your thyroid actually produces
- Free T3 (optimal: 3.0-4.0 pg/mL): The *active* thyroid hormone that drives metabolism. Many patients have normal T4 but poor conversion to T3
- Reverse T3 (optimal: below 15 ng/dL): A metabolically inactive form that can block T3 receptors. Elevated in chronic stress, caloric restriction, and illness
- Thyroid antibodies (TPO and TgAb): Detect Hashimoto's thyroiditis, the most common cause of hypothyroidism, which can be present for years before TSH becomes abnormal

Chronic inflammation is the common thread linking heart disease, diabetes, autoimmune conditions, cancer, and neurodegenerative disease. Standard panels rarely assess it.
What to test:
- hs-CRP (optimal: below 1.0 mg/L): High-sensitivity C-reactive protein is the most validated marker of systemic inflammation and cardiovascular risk
- Homocysteine (optimal: below 8 μmol/L): Elevated levels indicate methylation dysfunction, B-vitamin deficiency, and increased cardiovascular and neurological risk
- ESR (Erythrocyte Sedimentation Rate): A general inflammation marker, useful when tracked over time
- Ferritin (optimal: 50-150 ng/mL for women, 75-200 ng/mL for men): Both a marker of iron storage AND an acute-phase reactant (elevated in inflammation)
- ANA (Antinuclear Antibodies): Screens for autoimmune activity, which can smolder subclinically for years before a diagnosis
Hormones
Unless you specifically request them, most doctors don't order hormone panels until you're already symptomatic. By then, dysfunction has been developing for years.
For women:
- Estradiol, progesterone (timed to cycle day): Detect estrogen dominance, progesterone deficiency, and perimenopause
- Total and free testosterone: Yes, women need testosterone too — low levels cause fatigue, low libido, and muscle loss
- DHEA-S: An adrenal androgen precursor; low levels suggest HPA axis dysfunction
- Sex Hormone Binding Globulin (SHBG): Affects how much hormone is bioavailable; elevated by birth control, low in insulin resistance
For men:
- Total and free testosterone (optimal total: 600-900 ng/dL): Standard "normal" starts at 264 ng/dL — a level at which most men are symptomatic
- Estradiol: Yes, men need this tested too — elevated estrogen in men drives weight gain, gynecomastia, and cardiovascular risk
- DHEA-S: Adrenal function indicator
- SHBG: Determines free testosterone availability
- PSA: Prostate screening baseline
For everyone:
- Cortisol (morning fasting or 4-point salivary): Assess HPA axis function and stress response
- Pregnenolone: The "mother hormone" — low levels suggest hormonal raw material depletion
Advanced Cardiovascular Risk
A standard lipid panel tells you your total cholesterol, LDL, HDL, and triglycerides. That's a start, but it misses the markers that actually predict heart attacks.
What to test:
- Advanced lipid particle testing (NMR LipoProfile): Measures LDL particle *number* and *size*. Small, dense LDL particles are far more dangerous than large, buoyant ones — but a standard LDL number doesn't distinguish between them
- Lipoprotein(a) [Lp(a)] (optimal: below 30 mg/dL): A genetically determined cardiovascular risk factor present in 20% of the population. It should be tested once in every adult's lifetime — and almost never is
- ApoB (optimal: below 80 mg/dL): Arguably the single best predictor of cardiovascular risk, representing the total number of atherogenic particles
- Oxidized LDL: Measures the LDL that's actually causing arterial damage
- Triglyceride-to-HDL ratio (optimal: below 2.0): A simple calculation from standard labs that strongly predicts insulin resistance and cardiovascular risk
Lipoprotein(a) is genetically determined and present in 1 in 5 people. It dramatically increases heart attack and stroke risk, yet most people have never had it tested. It only needs to be checked once. If you've never had it measured, ask for it at your next visit.
Micronutrients & Vitamins
Deficiencies are far more common than most physicians realize, and standard panels rarely test for them.
What to test:
- Vitamin D (25-OH) (optimal: 50-70 ng/mL): Deficiency affects immunity, mood, bone health, and cancer risk
- Magnesium RBC (optimal: 5.5-6.5 mg/dL): Serum magnesium is nearly useless — it reflects only 1% of total body stores. RBC magnesium is far more accurate for detecting the widespread deficiency that drives insomnia, anxiety, muscle cramps, and arrhythmias
- Ferritin (see inflammation section): Low ferritin causes fatigue and hair loss long before hemoglobin drops
- B12 (optimal: 500-1000 pg/mL): Standard "normal" starts at 200, but neurological symptoms can appear below 400
- Folate (RBC): More accurate than serum folate for assessing true tissue levels
- Zinc (RBC preferred): Important for immunity, hormones, and wound healing
- Omega-3 Index (optimal: above 8%): Measures EPA+DHA in red blood cell membranes; reflects long-term omega-3 status and cardiovascular risk
Gut Health Markers
When GI symptoms, autoimmune conditions, or unexplained inflammation are present, deeper gut investigation is warranted:
- Comprehensive stool analysis (GI-MAP or GI Effects): Assesses microbiome diversity, pathogenic organisms, digestive enzyme function, and inflammatory markers
- Zonulin: A marker of intestinal permeability ("leaky gut")
- Calprotectin: Distinguishes inflammatory bowel conditions from functional GI issues
- Food sensitivity testing (when clinically indicated): IgG and IgA panels can identify trigger foods, though elimination diets remain the gold standard
How to Talk to Your Doctor About Advanced Testing
Here's the practical reality: most conventional physicians won't order all of these tests. Some won't be covered by insurance. Here are your options:
1. Bring a specific request: Write down the tests you want and explain why. Many physicians will order them if asked directly

Having advanced labs is only useful if they're interpreted through a functional lens. Standard labs compared against population-based reference ranges will yield the same "everything's normal" conclusion. Functional interpretation considers:
- Optimal ranges rather than standard reference ranges
- Patterns across markers — for example, high fasting insulin + elevated triglycerides + low HDL together signal metabolic syndrome, even if each marker individually falls within "normal" range
- Trends over time — a rising TSH from 1.5 to 3.8 is significant even though both values are "normal"
